VERIFICATION OF TREATMENT NOTES
Students who desire a note regarding their treatment received in the SHC must present themselves for evaluation at the SHC on the day they are ill or injured. Backdated notes will not be issued except in extreme circumstance. A verification of treatment note is not an excuse for absence; it is only a verification that the student received treatment. If a student leaves campus to see their primary healthcare provider at home and misses class doing so, they must receive a signed note from that provider. The nature of the student's illness will not be disclosed to the instructor without the permission of the student.
